1. Supervision and Responsibility

By signing this waiver, I acknowledge that Kreative Play Learning Center is an indoor play and learning environment. I agree that all children under my care must be accompanied and directly supervised by a responsible adult (18+) at all times. Kreative Play staff is present to facilitate programs, not to provide individual childcare or supervision.

2. Rules of Play & Safety

I agree that all participants (children and adults) must follow the center's rules, including but not limited to: wearing clean socks in the designated play zones at all times, no rough play, and respecting other children. Kreative Play reserves the right to ask anyone violating these rules to leave without a refund.

3. Assumption of Risk & Liability

I understand that active indoor play involves physical activity, including climbing, sliding, and jumping, which carries inherent risks of minor injuries. I voluntarily assume all risks associated with my child’s participation and hereby release, waive, and hold harmless Kreative Play Learning Center, its owners, and employees from any claims, liabilities, or injuries arising during our visit.

4. Health and Wellness Policy

I certify that the children listed below are healthy and free from contagious illnesses. If a child shows symptoms of sickness, I understand they will not be permitted in the play areas for the safety of the community.
